AMENORRHOEA, OLIGOMENORRHEA, IRREGULAR MENSTRUATION:

Amenorrhoea means the absence of menstrual periods. Primary amenorrhoea refrs to the condition where menstruation has not begun, although the girl has reached or passed the age when it should have commenced. It may be indication of a systematic disorder or of hormonal imbalance. Secondary amenorrhoea refers to the condition where a woman of childbearing age, who previously had periods has now stopped having them. If this is not due to pregnancy or menopause it must be regarded as a symptom requiring further investigation, as it is a feature of a wide range of conditions. Common causes of this type of amenorrhoea are anemia, debilitating diseases, hormonal imbalances, nervous strain and other emotional disorders. Since it is a symptom, it is necessary that the underlying condition be elicited and treated.

Oligomenorrhoea is a form of amenorrhoea where there is only an occasional or scanty flow.

The term irregular menstruation is used to describe cases where the periods occur, but on an abnormal manner, e.g., irregularlity of the menstrual cycle, excessive or scanty, prolonged or brief bleeding, or the blood being too dark or clots being formed.

Dysmenorrhoea

This refers to excessive pain during the periods. It is felt mainly in the pelvic area, lower abdomen and lower back. Primary dysmenorrhoea, which is the more common type, affects girls from the first period or soon after. Neither the cause nor the source of the pain s known and there is no observable pelvic lesion.However, it is likely that there may be an imbalance of th sex hormones in such cases.

Secondary or symptomatic dysmenorrhoea occurs in women who have previously had normal periods; it may be caused by a variety of gynecological disorders.

Menorrhagia; Functional Uterine Bleeding

Menorrhagia or excessive loss of blood during the monthly periods is frequently due to local disorders, such as fibroids. It may also, not uncommonly, be brought on by anxiety and other emotional factors. In severe cases, or if prolonged, it may cause anaemia. It is usually found in women over the age of forty. The possibility of a neoplasm must be excluded.

Leucorrhoea

This is an excessive discharge of a white or colourless secretion from the vagina. It is a symptom of irritation caused by infections such as candida or trichomonas.

Salpingitis, Adnexitis, Cervicitis, Vaginitis and other Pelvic inflammations

Acute or chronic inflammation of the Fallopian tubes (salpingitis), inflammation of the structures in close proximity to the Fallopian tubes, e.g., ovaries (adnexitis uteri), inflammation of the neck of the womb (cervicitis), inflammation of the vagina (vaginitis) are all the resullt of infection, usually by bacteria which usually spreads upwards from the vagina.

Uterovaginal Prolapse

In this condition, the vagina and also the uterus prolapses through the vaginal orifice. It is a common disorder of elderly women due to the weakening of the supporting ligaments and pelvic muscles by frequent pregnancies, chronic cough, or other participating causes. Since this is basically a mechanical disorder, relief can be obtained with surgery or a pessary.

Pruritus Vulvae

Pruritus or itching of the vulva frequently accompanies neuro-dermatitis. It may be due to local ccauses, e.g., worm infestation, vaginitis, and it may arise as a complication of the menopause.

Obstetric Disorders

HYPEREMESIS:

This term describes the excessive vomiting in pregnancy which may occur in the first trimester. The cause is not known, but psychological  factors are probably involved  in very severe cases, the pregnancy may have to be artificially terminated.

Although acupuncture is generally contraindicated in pregnancy owing to the risk of abortion, it may be used with advantage in cases of hyperemesis as it is less damaging to the fetus than drugs.

Malposition of the fetus

The correction of the malposition or malpresentation of the fetus in the pelvis historical procedure in traditional Chinese medicine.This condition is caused by many factors. Causes such as contracted pelvis, tumours of the pelvis and congenital abnormalities of the uterus should be managed by other measures.

The procedure has to be repeated daily until the fetal position is corrected.

Relief of pain during delivery

Acupuncture may be used during childbirth to relieve the pains of delivery. It may also be used in the other events that may occur during delivery such as delayed labour, episiotomy, suturing of the perineum after delivery, caesarean section and the manual removal of placenta

Mastitis

Acute mastitis or inflammation of the breast is commonly found in the period just after the birth of the baby, in association with a cracked nipple with added infections.

Symptoms:

Breast pain and fever, with the infected nipple becoming red and tender. If the fever continues for more than 48 hours, the affected breast swells with the enlargement of the axillary lymph nodes.

Chronic mastitis sometimes occur when an acute infection of the breast has failed to resolve completely.
